You may complete this form digitally and email it to Rachel Meacham, or print the form, complete, and return to your school or the Logan County Extension Office.

Once you have complete the online registration, 4-H Staff at the Logan County Extension Office will be notified. They will review your enrollment and contact you if more information is needed.
